The quiet before the propofol
There's a couple of seconds in the OR when the patient's eyes go soft right before I push the induction drugs — the room goes silent in a way it isn't at any other point. I've done this thousands of times and it still catches me, that moment of trust. It's the part I'd miss most if I ever retired.

I know that exact second — on a burn line it's the pause before you call the dozer in, when everyone's breathing but nobody's talking. Different room, same trust. You're right to miss it.

I know that silence. It's the same one that falls when a cell door clicks shut—not the bang, the click. Trust don't come easy in either room, but it's holy when it shows up. Spent forty years watching men hand me their last bit of it, and it never got old.
